Step 2. Find a Health Care Provider
Our staff of highly skilled Health Care Providers provides quality care across a wide range of services.
We offer services such as immunizations, pediatrics, prenatal care, dentistry and more to get you healthy and keep you that way. We have Health Care Providers at our centers who will meet your needs. Identify a health care provider that you feel comfortable with at the site identified in Step 1.
Step 3. Schedule your Appointment
Once you've located your center and chosen the Health Care Provider you'd like to see,
contact us at 1-877-809-5092 to schedule an appointment.
Attending to your needs in a timely manner is important to us, so
appointments often are available later the same day or within 1 to 2 days. If
you must cancel or reschedule, please provide us with at least 24 hours
notice. Walk-in patients may be seen, depending on Health Care Provider availability.
Please plan to arrive 15 to 20 minutes before your scheduled appointment
time so that we can update your medical records.
Step 4. Your First Visit
- Photo ID
- Your insurance card
- List of prescriptions and current medication bottles
- Current immunization records
- Any discharge paperwork from any hospital visits
Healthcare for Uninsured Patients
Financial Support Application Process
Adelante Healthcare evaluates each uninsured patient based on specific
qualifications. These include family income, size, and situation to determine
discounted services. We also facilitate application for additional discounting
services such as state Medicaid (AHCCCS) before scheduling an appointment.
All applications remain confidential and patients will be notified of all decisions.
To start the application process, please review the documents required for
financial support application by clicking on the link below (registration forms) for Step 5
and select the Uninsured Financial Support Form . You will need to schedule
a Financial Support appointment with us to finalize your application. After the
Financial Support appointment, you can schedule your first medical appointment.
Step 5. Registration Forms
To expedite the process, please review our registration forms prior to your
arrival and, if possible, download , print them, and fill out to bring them to
your appointment.

5092 to leave a message for the Health Care Provider on call. Dial 911 for life-threatening
emergencies.
